DOH Sarasota Community Well-Being Survey

The Florida Department of Health in Sarasota County (DOH-Sarasota) invites residents to complete the Community Health Well-being Survey. The 2024 Community Health Well-being Survey collects data from Sarasota County residents on nutrition, physical activity, access to care, and mental health. The survey is available online at surveymonkey.com/r/X6SDWMZ. This survey is only for Sarasota County residents that are 18 years of age or older. All survey responses are anonymous and no identifying information is collected. The survey takes approximately 10 minutes to complete. Data collected from the Community Health Well-being Survey will help DOH-Sarasota better understand the needs of the community and will be used to develop a community health improvement plan, addressing the issues residents are most concerned about. The Sarasota County Community Health Well-being Survey results will be shared with the community on the DOH-Sarasota website. The most recent Sarasota County Community Health Well-being Survey was performed in 2019.
